1. The undersigned knowingly and fully assumes all risks, known and unknown, associated with participation in activities at NAZOVI INC DBA Kids Town, Indoor Playground facility, and waives all claims for damage to personal property or injury to person arising from such participation. This release and waiver covers risks of death, serious injury, and property loss whether arising from (a) negligence or carelessness on the part of the persons or entities being released and other participants, or (b) dangerous or defective equipment.
2. Acknowledging participation in the activities at NAZOVI INC DBA Kids Town, (the Company) facility, the undersigned, on his or her own behalf, and on behalf of any executors, heirs, successors and assigns, hereby acknowledges and agrees as follows:
3. The undersigned certifies that he or she is physically fit and may participate in the activities available at an NAZOVI INC DBA Kids Town, Indoor Playground facility, and has not been advised to the contrary by a qualified medical professional;
4. The undersigned on his or her behalf and on behalf of any executors, heirs, successors and assigns, hereby releases, discharges, and holds harmless any authorized entity doing business as NAZOVI INC DBA Kids Town, including its officers, agents, employees, representatives, and all affiliates, from any and all claims, damages, or liability arising from death, disability, personal injury, property damage, or theft, or any kind of action;
5. The undersigned agrees to abide with the rules and conditions of participation posted at or explained by an employee at a NAZOVI INC DBA Kids Town, Indoor Playground facility. If the undersigned experiences any significant hazard or injury during such participation, the
undersigned will promptly inform an employee of the Company facility which they are currently attending; and
6. The undersigned acknowledges that this release and waiver of liability form will be used and relied upon by the Company, and that it will govern the undersigned’s actions and rights.

The undersigned parent and/or guardian does hereby represent that he/she is, in truth, acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above of all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act, and releases said parties on behalf of the minor and the parents or legal guardian.
7.I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
8.I further acknowledge that NAZOVI INC DBA Kids Town has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
9.I further acknowledge that NAZOVI INC DBA Kids Town can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result
from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.
10.I voluntarily seek services provided by NAZOVI INC DBA Kids Townand acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
11.I attest that:
* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
12.I hereby release and agree to hold NAZOVI INC DBA Kids Townharmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from NAZOVI INC DBA Kids Town. I understand that this release discharges NAZOVI INC DBA Kids Town from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from NAZOVI INC DBA Kids Town
13. This liability waiver and release extends to the playground together with all owners, partners, and employees.
